Morgan Records Management is now overseeing patient records custodianship and releases. Please know that patient confidentiality is always a priority. Patient releases will be handled respectfully and in accordance with HIPAA regulations.
Dear Patients,
Dr. Burnham’s Neurology Practice officially closes on December 31, 2021. Prior to the closing date, if you need your records for an appointment with a new neurologist, then contact the office at 303 788-7440. You can then either have the records faxed to the physician or pick up your records in the office. After the closing date of the practice your Medical Records will be available at Morgan Records Management via the links on this page.
Notes For Patients
if you only require your EMR record please make note of that in the remark/comments field under reason for request on the request form. This will help expedite the process for a quicker file transfer.
